MEMBERSHIP APPLICATION MEDICAL RELEASE WAIVER LOCKER ROOM RULES
CODE OF CONDUCT PARENT CONTRACT
PHYSICAL EXAMINATION / PARENTAL CONSENT FOR SWIM
All swimmers, new and returning, must complete the following forms to register for
the Naples Tigersharks Swim Team.
- Medical Release form
- Swimmer's Code of Conduct form
Please be sure to read over the rules with your swimmer(s) so they will know what is expected of
them.
- New swimmers will need to fill out the Membership Application.
- Every swimmer will need to have a Sport's Physical.
**If you are a high school student and hope to earn a varsity letter in swimming this
season, you must include a copy of your current physical in your registration
paperwork.
Please mail them MPS to: Membership Coordinator
